A Medicare annual review is easier and more useful when you arrive prepared. You do not need a perfect file or a box full of paperwork. You need a short, current picture of what matters to you.
Medicare says that people enrolled in a Medicare plan should receive an Annual Notice of Change each fall. The notice explains changes in coverage, costs, and other plan details that take effect in January. Reviewing that notice is an important first step, even when you have been happy with your current coverage.
Your preparation checklist
Bring the Annual Notice of Change from your current plan. If you have the Evidence of Coverage, bring that too. Mark any section that is confusing or important to you. If you did not receive your Annual Notice of Change, Medicare recommends contacting your plan.
Write down each doctor's full name and the exact office or facility location you use. Include your preferred hospital, specialists, therapy locations, laboratories, and other facilities that matter to your care.
For each prescription, write down the name, dosage, and how often you take it. Include the form when it matters, such as a tablet, injection, cream, or inhaler. Medicare's plan comparison tools can use saved drugs and pharmacies to estimate plan costs.
List your regular local pharmacy, any preferred pharmacy, and any mail order option you use or want to consider. Medicare explains that pharmacy networks and preferred pharmacies can affect what you pay.
Write down any change that may affect what you need. Examples include a new doctor, a new prescription, more travel, a move, caregiver involvement, language support, accessibility needs, or a change in how you prefer to receive care. Also note what is working well.

- Are my doctors and exact locations part of the relevant network?
- Are my prescriptions covered, and what are the expected costs?
- Does my pharmacy choice affect those costs?
- What out of pocket costs should I compare beyond the premium?
- What happens when I travel or need care away from home?
- What changed for the coming year?
- What would stay the same if I make no change?
A review does not automatically mean a change
Coverage should be reviewed against your real life, not only against a monthly premium. A good review may confirm that your current direction still fits. The purpose is to understand the current information, document what matters to you, and make an informed decision.
